Website Feedback
Feedback
Capture the page URL, the task a visitor was finishing, and what blocked them on the site.
Homepage analytics tell you bounce rate, not that the pricing table hid the annual toggle. This template asks which URL they were on, what they were trying to finish, and whether they actually finished it.
Who it is for
Marketing and web teams who still treat “contact us” as their only site feedback path.
When to use it
Float it as a small link in the footer or a docs article. Product Feedback is for the app. Support Request is for defects you can reproduce. Contact Us is a mailbox, not a task study.
What this form collects
Optional identity, a page URL, the intended task, a findability star rating, whether they finished, and an optional blocker note.
- Pricing-page “was this clear?” widget
- Docs article footer
- Post-redesign crawl of top landing pages

Frequently asked questions
Why require a URL?
“The site is confusing” is not actionable. A path lets you open the same page the visitor saw.
Can I prefill the page they came from?
Yes. Add a hidden field named from the query string after you clone the template, or describe that prefill to Customize with AI.
Should this replace heatmaps?
No. It explains the sessions heatmaps cannot narrate. Keep both if you already pay for a click tool.
