NPS Survey
Feedback
Measure loyalty with a 0–10 radio recommendation score and a follow-up comment.
Net Promoter is one question with a reason. This template keeps the 0–10 radio, asks why, and only then offers an optional email for follow-up.
Who it is for
Founders and CX leads who want a quarterly loyalty read they can compare over time.
When to use it
Use it as a relationship survey, not a ticket closer. Customer Feedback is better for a single interaction. Employee Engagement is the internal cousin.
What this form collects
A required 0–10 recommendation score, an optional reason, and an optional email.
- Quarterly customer pulse
- Post-onboarding loyalty check
- Board-level CX snapshot

Frequently asked questions
Why is the score a radio instead of stars?
Classic NPS is 0–10, not a 5-star scale. Stars hide the promoter / passive / detractor cut that most teams still report.
Can I tag the survey by plan or cohort?
Add a hidden field or a dropdown after you clone it. Do not put account IDs in the visible question unless respondents already know them.
Does GRTforms compute the NPS formula?
Submissions store the raw score. Export or connect the list to the spreadsheet or warehouse where you already calculate promoters minus detractors.
