Support CSAT
Feedback
Score a closed support interaction with ticket context, channel, and whether the issue is actually done.
Closing a ticket is not the same as the customer feeling helped. A CSAT that only asks for stars cannot tell you the issue is still open in their head. This template ties the score to a ticket number, a channel, and a resolved-or-not radio.
Who it is for
Support leads who want a post-close pulse that still points at a ticket, not a brand-level loyalty score.
When to use it
Send it from the resolve email. Customer Feedback is a general experience pulse. NPS Survey is quarterly loyalty. Support Request is how the ticket should have been opened.
What this form collects
Ticket email, ticket number, channel, a helpfulness star rating, whether the problem is actually done, and an optional “what would have been faster” note. A hidden ticket source can ride in from the email link.
- Zendesk or Intercom close survey stand-in
- Phone-queue follow-up SMS link
- Community-moderator quality check

Frequently asked questions
Does a low score reopen the ticket?
Not automatically. Route “still need help” to a human. The form is a score, not a workflow engine.
Can guests try the layout before I publish?
Yes. The template page preview is a live read-only render. Customize with AI opens the guest builder with this CSAT preloaded.
Why not ask if they would recommend us?
That question belongs on NPS Survey. Mixing loyalty into a ticket closer pollutes both numbers.
