Let a person or foundation start a scholarship fund with amount intent, criteria, and whether the gift is named.
Scholarship gifts stall when a donor emails “we want to help students” with no amount and no criteria. This template records an intent and the eligibility story so advancement can draft guidelines — it does not award a student and it does not process a payment.
Who it is for
College advancement shops, community foundations, and high-school booster funds.
When to use it
Use it when the visitor is creating or adding to a fund. Student Enrollment is the student path. Grant Interest is an org asking you for money. Donation Pledge is a general gift intent without scholarship rules.
What this form collects
Donor identity, optional fund name, amount intent, structure, eligibility notes, naming preference, and optional memorial instructions.
- Advancement “create a scholarship” page
- Community-foundation donor-advised conversation starter
- Memorial fund after a student or teacher death

Frequently asked questions
Does this select a student?
No. Students apply through your existing scholarship process. This form is the donor side.
Does this take the gift?
No. It is an intent. Completing the gift still happens by check, stock, DAF, or the payment page you already run.
Can a company endow a scholarship here?
Yes — put the company in the story or add a company field after you clone it. Sponsorship Request is the better path if they want a logo on a banquet, not an award criterion.
