Retail Visit Feedback
Feedback
Record whether a shopper found what they came for, plus staff and floor findability at a named store.
A store visit that ends with “we could not find it” never shows up in the ecommerce dashboard. This template asks which location, whether they left with what they came for, and which item was missing on the floor.
Who it is for
Retail ops and brand field teams who still run stores as well as a site.
When to use it
QR at the exit or on the receipt. Restaurant Feedback is food service. Product Feedback is the SKU itself, not the aisle. Customer Feedback is a brand-level pulse without a store ID.
What this form collects
Store name, visit date, staff and findability stars, whether they left with the item, an optional missing SKU, and optional notes.
- Receipt QR after checkout
- Mystery-shop complement for a district manager
- Pop-up shop debrief the same weekend

Frequently asked questions
Can I use store numbers instead of names?
Yes. Change the location field to a dropdown of store IDs after you clone it so associates cannot invent a site.
Should associates fill this for the customer?
No. Hand them the QR. Associate-completed cards read like a self-grade.
How do I attach this to a receipt?
Add a short-text receipt field, or prefill a hidden field from the POS link. Keep it optional so cash customers can still talk.
