IT Access Request
Admin
Request an account or permission with system, access level, and a manager who will sponsor it.
An access ask that is only an app name is how admin roles leak. This template asks for a level, a sponsor, and a reason so IT is not guessing whether they meant read or owner.
Who it is for
IT and identity owners who still grant access from a shared inbox.
When to use it
Use it for accounts and groups. Asset Checkout is hardware. Key Fob Request is a door. Employee Onboarding is the whole first-week packet.
What this form collects

Frequently asked questions
Does this create the Active Directory account?
No. Someone still clicks provision. The form is the ticket with a sponsor already named.
Can I time-box contractor access?
Use Needed by as the start, and add an end-date field after you clone it if contractors are common.
Where does offboarding go?
A different form or a checklist. This one is a grant. Do not overload it with “remove me from everything.”
