When to use it
Use it for planned work. Support Request is a defect that already exists. IT Access Request is an account grant. Time-Off Request is a person being gone, not a system changing.

Does this approve the change?
No. An owner still says yes. The form is the request so the first message already has risk and a date.
Where do emergency changes go?
A phone tree. Use this for work you can still schedule. Add an “emergency / already done” radio after you clone it if you want a paper trail after the fact.
How is this different from a support ticket?
Support describes something broken. A change describes something you intend to alter. Keep the verbs honest so the wrong queue does not own Friday night.
