Asset Checkout
Admin
Check out a laptop, kit, or key with asset tag, return date, and why it is leaving the closet.
A laptop that “someone grabbed” has no return date. This template asks for a tag, a promised day, and a reason so IT is not hunting a serial across Slack.
Who it is for
IT closets, theater shops, and field teams who still track gear in a spreadsheet.
When to use it
Use it when the item leaves the room. IT Access Request is an account, not a brick. Key Fob Request is building access. Change Request is a system change.
What this form collects
Borrower identity, asset kind, tag or serial, return date, optional manager, and a reason.
- IT loaner laptop closet
- AV kit for an off-site
- Facilities tool crib

Frequently asked questions
Does this update our inventory database?
No. Export or copy the row. The form is the receipt, not the CMDB.
Can someone take more than one item?
Submit once per tag, or add a second asset-tag field after you clone it. One serial per row keeps overdue lists honest.
What happens if they keep it past the return date?
You chase them. Add a late-note field if you want them to admit it on a follow-up form.
