Employee Referral
HR
Let employees refer a candidate with the role, their relationship, and why they would work together again.
Referrals dropped in Slack lose the relationship and the role. This form makes an employee name the candidate, the job, and why they would sit next to them again.
Who it is for
People teams that pay a referral bonus and still need a paper trail.
When to use it
Use it on the intranet when the submitter is a current employee. Job Application is the candidate applying themselves. Intern Application is campus. Candidate Screening is a recruiter-facing pre-screen.
What this form collects
Employee identity, candidate name and email, role family, relationship, and a required why.
- Intranet “refer someone” page
- Referral sprint during a hiring push
- Alumni-employee introductions

Frequently asked questions
Does this start the bonus clock?
Only if your policy says a form submission counts. Recruiting still has to confirm the hire. Say that in the success copy.
Should the candidate know they were referred?
Most teams tell them. Put that expectation in the intro so employees do not surprise a friend.
Can I attach a résumé for them?
Add a file field after you clone it if employees already have a PDF. Otherwise send the candidate Job Application.
