Order Form
Sales
Business order form with product selection, quantity, and shipping details.
Not every order belongs in a cart. This template is for B2B or made-to-order requests: who is buying, which SKU, how many, and where it ships.
Who it is for
Wholesalers, studios, and ops teams who confirm availability before they invoice.
When to use it
Use it when payment happens later. Sample Request is the “send me one” cousin. Wholesale Inquiry is earlier in the relationship.
What this form collects
Company and contact, email, phone, product dropdown, quantity, shipping address, and special instructions.
- Trade customers reordering a known SKU
- Print shop or merch restock
- Internal supply request that looks like an order

Frequently asked questions
Does this template take payment?
No. There is no shipped payment field. Use this as the request, then invoice or checkout in the system you already trust.
Can I replace Product A/B/C with my catalog?
Yes — that dropdown is a placeholder. Edit the options in the builder or describe your SKUs to Customize with AI.
What if a buyer needs multiple products?
Ask them to submit once per SKU, or add repeating quantity fields after you clone. A long-text “line items” box also works for messy orders.
