No. It asks whether they can complete one. Your screening vendor still does the check after you invite them.
Should mentees use this form?
No. Build a separate youth or parent form. This one is written for the adult mentor.
Can I ask for a résumé?
Add a file field after you clone it if you want one. Many strong mentors do not have a youth-work résumé — the why field is the better filter.
