Rate a dated event with separate scores for the session and the logistics, plus which talk to keep.
Conference evals that only rate “the event” bury the session that ran long and the coffee that ran out. This template splits program quality from logistics and asks which talk to keep on the recording page.
Who it is for
Conference producers, meetup hosts, and internal offsite owners who need a same-week readout.
When to use it
Send it the evening of, while the room is still in memory. Course Evaluation is for a multi-week class. Webinar Signup is the registration side. Restaurant Feedback is for a dining room, not a ballroom.
What this form collects
Event name, date, format, session and logistics star ratings, an optional session to keep, and optional notes.
- User conference closing email
- Monthly meetup QR on the way out
- Internal all-hands afterparty debrief

Frequently asked questions
Can speakers see their own scores?
Not from this form. Export and share the slice you promised them. Do not put speaker-identifying comments on a public screen.
Why two star fields?
A great talk in a freezing room averages to “fine.” Split scores let facilities and programming fix different problems.
Should I collect dietary notes here?
No. Those belong on Event Registration before the catering order. This form is after the fact.
