Does this excuse the absence automatically?
No. Attendance still applies your policy. The form is a notice so the teacher is not guessing.
Can a student submit this without a guardian?
Yes for adult programs — relabel the name fields after you clone it. For minors, keep a guardian contact.
What about multi-day illness?
The return date is optional. Ask families to send a second notice if the plan changes instead of editing the first one.
