Scholarship Application
Education
Intake award applications with the student, the fund they want, a need or merit statement, and a transcript upload.
Scholarship inboxes that only ask for an essay get novels with no fund name. This GRTforms template asks which award they mean, what the transcript actually shows, and a statement long enough to score.
Who it is for
Foundation desks, PTA award committees, and college aid offices that still read applications in a shared folder.
When to use it
Open it for a dated cycle. Student Enrollment is the seat. Webinar Signup is an info session about the award, not the application itself.
What this form collects
Applicant identity, school and year, fund choice, optional GPA and credits, a required statement, and an optional transcript.
- Local foundation spring cycle
- Departmental merit award
- First-generation or need-based campus fund

Frequently asked questions
Does this decide who wins the award?
No. Reviewers still read and vote. The form is the packet so you are not assembling PDFs from five email threads.
Should GPA be required?
Keep it optional if unofficial transcripts vary by school. The upload is the source of truth; the typed GPA is a sort key.
Can applicants apply to more than one fund?
They can submit twice, or you can turn the fund dropdown into checkboxes after you clone the template. One statement per fund is easier to score.
