Referral to Specialist
Healthcare
Start a specialist referral with specialty, urgency, and what the receiving office should know — a routing packet, not an order.
A referral that only says “please see ortho” arrives without a why and sits in a fax pile. This GRTforms template asks for specialty, timing, and the story the receiving desk needs. It is a routing packet — not an order to treat and not medical advice.
Who it is for
Primary-care coordinators, school-based clinics, and patients asked to “get the referral started” themselves.
When to use it
Use it when a human still calls the specialist. Consultation Booking is the specialist’s own intro form. Appointment Request is a slot you already control. Medical Records Request is the chart copy that often travels with the referral.
What this form collects
Requester identity, patient name, specialty, optional current clinician, urgency, optional deadline, optional file, and a required routing note.
- PCP “start a referral” link in the after-visit summary
- School clinic sending a student to a specialist
- Patient-initiated second-opinion routing

Frequently asked questions
Does this authorize the specialist visit?
No. Payers and the receiving office still decide. The form is how coordination gets a complete ask.
Can the specialist’s office use this as their new-patient form?
Point them at New Patient Intake or Consultation Booking. This template is the sending side.
Should I include a diagnosis code?
Only if a clinician is filling it. Patients should describe the problem in words. Codes are a chart task, not something this intake should demand.
