Prescription Refill Request
Healthcare
Take a refill ask with medication name, pharmacy, and how they want it filled — intake for the care team, not a prescription.
Refill voicemails that skip the pharmacy name bounce between the nurse line and the wrong store. This GRTforms template asks for the bottle name and where to send it. It is a request for review — not a prescription and not medical advice.
Who it is for
Primary-care and specialty desks that still take refill calls on a shared line.
When to use it
Publish it for established patients. New Patient Intake is a first file. Symptom Check-in is for a new problem, not a routine refill. Insurance Update is coverage, not the bottle.
What this form collects
Patient identity, medication name, pharmacy, fill method, optional days remaining and last-fill date, and optional notes.
- Nurse-line after-hours form
- Portal “request refill” when e-prescribe is closed
- Rural clinic that phones the pharmacy by hand

Frequently asked questions
Does submitting this refill the medication?
No. A clinician still reviews and, if appropriate, sends the order. Controlled or expired therapies may need a visit — that decision is not made by this form.
Can I request a dose change here?
Use the notes to flag a question. Dose changes are clinical decisions. This template is intake for a refill review only.
What if I do not know the last fill date?
Leave it blank. The pharmacy and chart usually have it. Days-left is the more useful urgency signal.
