Shop Purchase Order
Admin
Request a purchase with the part, quantity, vendor hint, and when the floor needs it — not an ERP.
A sticky note on a machine is not a purchase order. This template names the part, the quantity, and the need-by date so purchasing can source it. It does not issue a PO number by itself.
Who it is for
Shop leads and buyers in small plants that still email purchasing.
When to use it
Use it when the floor needs a part. Vendor Onboarding is a new supplier. Parts Return Request is sending something back. Machine Maintenance Log is the repair history.
What this form collects
Requester, part name, optional SKU, quantity, vendor hint, need-by date, quote radios, optional file, and a job note.
- Tool-crib replenishment
- One-off fixture buy
- Maintenance part that is not in the crib

Frequently asked questions
Does this create a PO in our ERP?
No. Purchasing still keys the PO. Export the submission or copy it into the system you already use.
Can I require a cost center?
Add a short-text field after you clone it if your plant codes jobs that way.
What if they do not know the SKU?
Keep it optional. A photo of the old part in the quote field is often enough to start.
